To arrange an Appointment

A referral letter is required in order to make an appointment with Dr Phillips. This can be obtained from your GP, dentist, or another specialist doctor. Once we have received this referral, it will be triaged by Dr Phillips and our team. We will then be in contact regarding an appointment. If you have not heard from us within 7 days, please phone us for an update.

Please forward your referral, including results of relevant scans, hearing tests or pathology, via email to admin@drdamienphillps.com.au 

Prior to your appointment

You will receive an SMS reminder in the days prior to your appointment to confirm the appointment. We ask that you respond with either YES or NO – if you do not confirm, your appointment time will be reallocated. Please double check the appointment location and time. Please allow sufficient time for parking as this can be time consuming and difficult around hospitals. 

Ensure that you have completed and returned the patient registration form that you have received from us. This is required prior to your appointment. If your referral information has only been received the day before your appointment it is a good idea to arrive 10 -15 minutes early to confirm and complete anything additional that is required prior to you appointment. Please contact us if you have not received any information since arranging your appointment time.

Ensure that any testing or scans requested by Dr Phillips or your referring doctor have been carried out prior to your appointment and have been received and added to your file.

On the day of your appointment

Please bring your Medicare card and Private Health Insurance information with you.

If the patient is under the age of 16 years, it is preferred for a legal guardian to accompany them to the appointment. If this is not possible, a further appointment may be required.

Please do not attend if you are unwell. Instead, contact us so that we can make other arrangements.
